Is It Really PCOS? The Hidden Impact of Hypothalamic Amenorrhea on Fertility
You’ve done the tests. The results came back “normal.” Maybe your doctor even mentioned PCOS. But something about that diagnosis doesn’t sit right.
If your cycles are missing, you’re constantly exhausted, and your lifestyle leans heavily into clean eating and daily workouts… there’s a real chance you’re actually dealing with Hypothalamic Amenorrhea (HA), a condition that’s frequently misdiagnosed or ignored altogether.
The PCOS vs. HA Mix-Up
Many women with HA are mistakenly told they have PCOS. Why?
- They’re not ovulating.
- Their cycles are irregular or missing.
- They may have multiple follicles visible on ultrasound.
But what doctors don’t always assess is why ovulation is missing. If hormones like LH, FSH, and estradiol are low, and your lifestyle involves under-fueling and high physical output, HA becomes a strong possibility.
We need to start ruling this out properly instead of jumping to the PCOS conclusive from lack of ovulation alone.
The Overlooked Culprit: Under-Eating and Over-Exercising
In podcast episode #165 (linked below), guest expert Victoria from Nourishing Minds Nutrition shares her clinical experience: most women with HA don’t even realize they’re under-eating.
We’ve been conditioned to believe that 1500–1800 calories is “plenty”… especially when paired with a lean protein + clean carb lifestyle. But for women who are active, this isn’t nearly enough to support basic hormone function, let alone fertility.
What to Watch For: HA Red Flags
You may be dealing with HA if you:
- Lost your cycle during a health or weight loss journey
- Are constantly tired, cold, or irritable despite eating “well”
- Work out daily and rarely rest
- Have a BMI on the low end, or have lost weight quickly
Why It Matters: Getting the Right Support
HA requires a completely different approach than PCOS. Standard PCOS interventions like low-carb diets or weight loss can actually worsen HA symptoms and delay conception further.
The goal with HA isn’t just to “get a period.” It’s to re-establish hormone signaling, repair your relationship with food and your body, and create a safe, supportive internal environment for pregnancy.
Inside Fertility Confidence Method, we assess for HA as part of your personalized fertility roadmap. We look at your full history, your lab data, your lifestyle patterns and we help you rebuild from a place of nourishment, not restriction.
